How To Become a Medical Marijuana Patient in the State of Arizona
Arizona residents with qualifying conditions can apply for a medical marijuana patient card in three steps. Before you begin the application process, it’s important to understand what medical conditions qualify for patients to receive an Arizona medical marijuana card, what the process entails and what fees are assessed.
Step One: Determining Eligibility
According to the Arizona Department of Health Services, patients with at least one of the following medical conditions can qualify for a medical marijuana card:
Post-Traumatic Stress Disorder
Human Immunodeficiency Virus (HIV)
Acquired Immune Deficiency Syndrome (AIDS)
Amyotrophic Lateral Sclerosis (ALS)
Agitation of Alzheimer's disease
A chronic or debilitating disease or medical condition or the treatment for a chronic or debilitating disease or medical condition that causes:
Cachexia or wasting syndrome;
Severe and chronic pain;
Seizures, including those characteristic of epilepsy;
Severe or persistent muscle spasms, including those characteristic of multiple sclerosis
Step Two: Approval Process
So how do you determine if you have a qualifying medical condition? The second step in the process is to contact your medical practitioner to see if medical cannabis can help. You will need current (within the last calendar year) medical records documenting your diagnosis with a qualifying condition.
If you have not been diagnosed with a qualifying condition, you will need to make an appointment with a licensed provider to formally obtain a diagnosis. Step Three: Fees
The licensed provider can charge between $75-$150 for an appointment where your qualifying condition can be approved.
Once your qualifying medical condition is approved, the cost to obtain your Arizona medical marijuana card is $150, which is valid for 2 years. If you participate in the SNAP program, the fee is $75. The licensed provider will submit the required documents to Arizona’s Department of Health Services. Once you’ve completed the process, your Arizona medical marijuana card will be mailed to you within 10 business days.
